According to Rutkow, orthopedic surgery as a specialty made its appearance in America in 1838 when John B Brown opened the Boston Orthopedic Institution - the first such specialized hospital in America. Twenty of the city's most prominent surgeons offered their services without fee at the hospital. Brown was responsible for developing and applying the surgical "remedy" for club-foot. He was the author of several papers on the specialty. Rutkow ORp38.

The author/cartoonist credits Raymond Chandler and Dashiell Hammett for "special inspiration," and occasionally uses quotes from both as captions or as the basis for his visual gags (single-panel cartoons). The author (per the rear-cover blurb) was a TV writer and stand-up comic; more recent information found online credits him with numerous short stories in the mystery, fantasy, science fiction, and horror genres, as well as his prolific cartooning work. .
