(ca 1899); 3 x 5; pp. 6; die-cuts to last leaf; a bit of wear and rubbing to fold lines, else bright and clean; name and address of, presumably, a person ordering the product typed; very good.A curious piece of advertisement, the item appears to have been an order form for Thymo Tooth Paste and Thymo Tooth Powder with die-cut slots for putting coins in and mailing it to order toothpaste. The slots are outlined and subtitled, according to the size of available coins (50 cents, nickel, etc.). Also included are descriptions of the products and several letters by physicians and specialists endorsing the use of those miracle items.
